What Can You Do with MyChart

You’ll need:
- Your MyChart activation code (from your clinic or text/email) - try signing up as soon as possible after receiving your code so that it doesn’t expire
- Your date of birth
- A smartphone, tablet, or computer
- Open the MyChart app
- Select your hospital/health system: NYC Health + Hospitals
- Enter your activation code
- Create a username, password, and security question
- Make sure your email address and phone number are yours, not your parent or guardian’s
Once You’re in, Start Taking Charge of Your Health
You can:
- View upcoming visits
- Schedule/select appointment times (if your clinic allows)
- Check in ahead of time through eCheck-In
- Complete forms or questionnaires before your visit
- Lab or imaging results usually appear within days
- Tap "Test Results" to read them
- Green check marks mean results are normal
- Sometimes results may be outside the normal range but can be normal variation; please try not to worry until you discuss your results with your provider
- It is possible that you may see your results before your provider sees them; please give them some time to contact you to explain the meaning of results
- You can message your provider with questions
Use MyChart to safely send:
- Medical questions
- Refill requests
- Concerns after a visit
MyChart is not for emergencies. It may take up to 72 hours for someone to see your message. If it’s urgent, call 911 or go to the ER.
Other care team members may review your message before your provider does.
How to send a message:
- Click "Messages" → select "Send a Message" → select "Ask a medical question" → select "Non-Urgent Medical Question" → select your provider's name
- See what medications you take
- Request refills
- Check instructions and doses
Great for school, sports, and travel.

- Access your full health record
If your provider offers telehealth:
- Make sure the MyChart app is updated
- Tap "Video Visit" in Appointments
- Allow camera + microphone when prompted
Tip: Join 10 minutes early to test your connection.
A MyChart Proxy is a parent or legal guardian who has access to some of the information in your MyChart account.
Teen patients ages 12 to 17:
- Some sensitive health information may be private from proxies
- Sensitive information (for example: appointments, medications and test results related to sexual and reproductive health) will be kept confidential and not shared with proxies
- You control your username and password; we encourage you to keep your password private and not share with others
- Your parent/guardian can have a separate proxy account and can view your non-sensitive health information through that account
Young adult patients ages 18+:
- You will need to sign a consent form if you would like your parent/guardian to continue to have access to view your health information through their proxy account
- If you grant proxy access to your parent/guardian, they will be able to see all health information (both sensitive and non-sensitive)
If you have questions about privacy, ask your provider.
- Forgot password → tap "Forgot Password"
- Locked out → call clinic MyChart Help Desk
- App glitches → delete and reinstall the app
